Paciente de 23 años de edad, con fecha del primer día de su última menstruación normal el día 28 de Septiembre de 2008 y es su segundo embarazo, después de 7 años. Se le practica una ecografía pélvica de control, observándose un saco gestacional con paredes íntegras y con un embrión de 3.6 milímetros en su interior, correspondiente a 6.1 semanas de gestación. El diámetro mayor del saco gestacional es de 28.2 milímetros. Se auscultaron los latidos cardiacos en 120 latidos por minuto y finalmente podemos ver el saco vitelino, lo cual indica un buen pronóstico junto la reacción positiva de la decidua (área hiperecogénica alrededor del saco gestacional). Patient 23-year-old of age, with date of the first day of her last normal menstruation on September 28, 2008 and it is her second pregnancy, after 7 years. A pelvic ultrasound scan of control is practised , a gestacional sac being observed with complete walls and with an embryo of 3.6 millimeters in the interior corresponding to 6.1 weeks of gestation. The biggest diameter of the gestacional sac is 28.2 millimeters. The cardiac beats were auscultated in 120 beats for minute and finally we can see the yolk sac, which indicates a good prognosis together with the positive reaction of the decidua (hyperechogenic area about the sac gestacional). Decidua is the term for the uterine lining (endometrium) during a pregnancy, which forms the maternal part of the placenta. It is formed under the influence of progesterone and forms highly-characteristic cells. Role:As the maternal interface to the embryo the decidua participates in the exchanges of nutrition, gas, and waste with the gestation. It also protects the pregnancy from the maternal immune system. Further, the decidua has to allow a very controlled invasion of the trophoblast. In invasive placental disorders like placenta accreta decidualization have been consistently found to be deficient. Hormone production: The decidua secretes hormones, growth factors, and cytokines. It has receptors for estrogen, progesterone, growth hormone, and others. Among its products are hormones commonly associated with other organs such as cortisol, CRF, GnRH, prolactin, and relaxin. Decidual prolactin is not under dopaminergic control. Pregnancy protein 14 (PP-14), also called placental protein 12, and Insulin-like growth factor-binding protein 1(IGFBP1) appear to be specific products of the secretory and decidual lining. Other factors released include interleukin-15 and vascular endothelial growth factor (VEGF). A reasonable understanding of the role and interplay of these hormones and factors has not been evolved.
