The most common cause of narrowing of the esophagus, or stricture, is scarring of the esophagus from reflux of acid occurring in patients with heartburn. Patients with a narrowed portion of the esophagus often have trouble swallowing; food feels like it is "stuck" in the chest region, causing discomfort or pain. Less common causes of esophageal narrowing are webs or rings (which are thin layers of excess tissue), cancer of the esophagus, scarring after radiation treatment or a disorder of the way the esophagus moves [motility disorder]. How Should I Prepare for the Procedure? An empty stomach allows for the best and safest examination, so you should have nothing to drink, including water, for at least six hours before the examination. Your doctor will tell you when to start fasting. Tell your doctor in advance about any medications you take, particularly aspirin products or anticoagulants (blood thinners). Most medications can be continued as usual, but you might need to adjust your usual dose before the examination. Your doctor will give you specific guidance. Tell your doctor if you have any allergies to medications as well as medical conditions such as heart or lung disease. Also, tell your doctor if you require antibiotics prior to dental procedures, because you might need antibiotics prior to esophageal dilation as well. What Can I Expect during Esophageal Dilation? Your doctor will then pass a tapered dilating instrument through your mouth and guide it into the esophagus. What Can I Expect after Esophageal Dilation? After the dilation is done, you will probably be observed for a short period of time and then allowed to return to your normal activities. You may resume drinking when the anesthetic no longer causes numbness to your throat, unless your doctor instructs you otherwise. It is set in a much simpler time of Mayberry-like-"aw-shucks" morality, in which people were polite and obedient. It was also the time of the Cold War's unique brand of mass paranoia and secrecy. Esophagus probes at the unspoken, taboo underbelly of this perfectly manicured society. The 1950's was a post-war decade full of saccharine, Technicolor optimism with under-riding hints of suspicion. It seems as though in our current era (which to some extent, may one day be regarded as a neo-1950's period, due in part to our present post-war condition, renewed obsession with procreation and distrust of foreigners), we are in a similar state, which is why Esophagus is relevant to today's viewer. I was influenced by science fiction films of the 1950's such as Don Siegel's Invasion of the Body Snatchers (1956), and Howard Hawks The Thing (1951), which both share a strong sense of paranoia within a society. People are taken over and act like drones, while the heroes' efforts seem futile as the problems become more widespread. However, the paranoia in Esophagus is reciprocal. Not only is the protagonist, Henry, suspicious of his wife's over-active salivary glands; but also the entire dinner party is repulsed and disbelieving of Henry's projectile vomitus. Thus the two parties find a conclusion in their mutual repulsion towards the drool or puke, respectively. I also drew from the oeuvre of David Lynch, especially his exploration of the underside of suburbia and his tone of domestic uneasiness. Many of his films are centered around somewhat dated, yet seemingly normal families such as the Palmers in "Twin Peaks" and the Beaumonts in Blue Velvet. Yet the suburban peace is undermined by the discovery of the macabre or surreal. In Esophagus, instead of a dead body or disembodied ear however, the pleasant status quo is disrupted as soon as Celeste serves Henry his after-work martini with a little too much slobber. The film then escalates to push Henry's (and the viewers') tolerance towards oral projections and the disruption of domestic decorum further. The resolution is reached only when Henry discovers that his affinity for vomiting far overshadows his distaste for his family and friends' nonchalant drooling. There is a cathartic purging sequence in which Henry's character is reborn through his vehement reaction at the dinner table. The film closes with Henry's rejection of society's regulations. In the great pastoral tradition, he returns to nature after struggling within the confines of a social structure. He casts off the restrictive garments of his conservative suit and leaps into the air and is frozen in a moment of perpetual delight. It Came From Beyond The Esophagus conveys its main themes of assimilation verses individuality, and domestic unease through the use of satirical allegory and visual humor. The film's message towards viewers is to be weary of the societal mold a generation may become confined to and to break free and become individuals, despite conventions and public reactions. (Less)
